This policy explains what information Gainesville Medical Obesity Specialty Clinic, LLC (“GMOS Clinic,” “we” or “us”) collects through this website, how we use it, and the choices you have.

This policy covers our website only. Your medical records and other health information are protected under HIPAA and handled as described in our Notice of Privacy Practices, which is available at the clinic and on request.

Information You Give Us

When you send a request through our contact form, we collect what you enter: your name, phone number, email address, preferred appointment date and time, and any message you write. If you call or email us, we receive the information you choose to share.

Please do not include medical details in the form or in email. Our website forms and regular email are not the place for health information. We will talk with you about your health by phone or in person.

Information Collected Automatically

Like most websites, ours collects some information automatically when you visit:

• Analytics. We use Google Analytics to understand how people use the site: pages visited, time on page, the type of device and browser, the website that referred you, and your approximate location (city level). This information is not used to identify you.

Children

This website is intended for adults. We treat some younger patients, but contact about a minor’s care should come from a parent or guardian. We do not knowingly collect information from children under 13 through this website.
